I’m incredibly grateful that people like Mr Aylott exist, we are so lucky to have them. I was mostly bed bound with back pain and lost control of my day to day life, I was unable to walk far, drive, sit, sleep well….I booked a one stop appointment with Mr Aylott at his clinic in Cheltenham, within less than 2 hours, I had received a consultation, an MRI, a diagnosis and a plan to get me back to me! Severe lower back pain diagnosed by Mr Aylott as fracture of T12 vertebrae and there was presumed osteoporosis. It was explained that the fracture would heal over time but the pain could be relieved by an operation involving cement augmentation (balloon kyphoplasty). No pressure whatsoever to have an operation but opted to do so. Pain relieved practically immediately after op at The Harley Street Hospital in London. Mr Aylott was very thorough, considerate and professional as were his team, the MRI scan staff, the staff at Spa Private Medical in Cheltenham and at the Harley Street Hospital. Also used the local recommended taxi service from home to the hospital. What is lumbar microdiscectomy surgery?
Microdiscectomy is one of the most routinely performed operations at The UK Spine Centre and lends itself to keyhole/minimally invasive surgery.
The procedure is done for people who have a herniated disc which is touching or compressing a nerve root in the spine. The surgery works by relieving pressure on the spine nerve root by removing/trimming the disc material which is causing the pain and symptoms in the back and leg.
Microdiscectomy differs from the old fashioned ‘discectomy’ which is still offered elsewhere. A discectomy involves more muscle and tissue damage and results in more pain and slower recovery.
At the UK Spine Centre a minimally invasive approach means, a much smaller incision, using specialised instruments, and the surgeon uses strong magnification and special lights to carry out surgery with extraordinary precision and focus leading to the best possible results and outcome for the patient.
Is a lumbar microdiscectomy major surgery?
A microdiscectomy is not considered ‘major’ surgery.
Mr Caspar Aylott uses minimally invasive modern techniques. Small incisions minimise tissue damage, reduce infection, reduce postoperative pain and speeds up recovery.
Unlike old fashioned surgeons we do not cause major tissue trauma or create long scars.
You will be asleep during the procedure but at the UK Spine Centre we use much lighter anaesthetic and regional anesthesia. This allows you to wake up feeling well and to be up and about and able to go home on the same day.
Patients are usually in theatre for approximately 1 hour and up and walking within 1-2 hours of surgery.

Sciatic leg pain is often immediately better following surgery. We encourage our patients to take regular short walks in the first two weeks building this up gradually.

Patients return to work and normal everyday activities as symptoms allow and depending on their occupation. Some patients will return to work at 2 weeks others may wait until 6 weeks.

The Problem
Sarah was a busy working mum living life to the full. In her 50’s, Sarah enjoyed her job as a primary school teacher, spending time with her husband and their two children and exercise was walking the dog. By the time she came to the UK Spine Centre she couldn’t walk, stand or sit without extreme pain. She was unable to work or function at home. Some good physiotherapy treatment and stretching had eased symptoms for a while but the sciatic symptoms had returned so severely, she had stopped work and the only relief she could get was lying in the foetal position.
Our Diagnosis
Sarah presented with pain in the buttock, going down the leg into the calf and foot with pins and needles and it was clear she had severe sciatica. The most common cause of sciatica is a disc herniation and although the majority of disc herniations shrink over time and symptoms settle, Sarah was still suffering at 6 months and sensible conservative treatment had not resolved the problem.
Reviewing Sarah’s MRI scan the cause of the sciatica was evident, a L5/S1 paracentral disc herniation compressing the traversing left S1 nerve root.
After this length of time the definitive solution was a microdiscectomy.
The Treatment
Sarah had her microdiscectomy 2 weeks after her initial consultation. She described it as ‘miraculous’. Within just a few hours of surgery she was walking pain-free and unaided. She had complete relief of both back and sciatic pain and had stopped taking any pain killers. By her 6 week follow-up consultation she was happily back teaching her busy reception class and had returned to all the activities she loved.
